How To Fill Out A Property Loan Agreement Renewal

This Knowledge Base article will show you how to fill out a Property Loan Agreement Renewal.

1) Once you have accessed the Property Loan Agreement form, select your campus from the drop down menu.

2) Input your personal information in all of the required text fields.

3) When filling out the "Hereinafter referred to as LENDEE for a period from" All renewals must have a TO (checkout end date) of 6/30/20XX, for the Current Fiscal Year.

4) Type in the reason as to why the item/equipment is being checked out to you in the "List Requested equipment and explain business purpose(s)" field. 

5) Ensure that the checkbox for the "Check here if this is a renewal:" is clicked and filled in. 

 

6) Input the tag number (located on the item/equipment). If there is not a Tag number type "N/A". A monitor would not have a tag number.

 

7) You must attach a photo of the physical tag number that is on the item/equipment. The Tag Number can be on the top or bottom of the laptop. 

8) Read the Terms and Conditions, before signing the renewal.​​​​

9) Click the (Click to sign) field. 

10) Type your first and last name in the appropriate field and click Sign Electronically

11) Submit the renewal Property Loan Agreement by selecting the Submit Form  button.
