How to Change Your MEID Password and Log Into Dynamic Forms

Introduction:

This document will provide you with instructions on how to reset your Maricopa (MEID) password and log into Dynamic Forms.

Changing your Maricopa (MEID) Password:

To reset your Maricopa (MEID) password follow the steps below:

 

Congratulations! You have now reset your Maricopa (MEID) password.

 

 

                 

 

Logging into Dynamic Forms:

 

 

 

 

 

 

 

 

6. Complete Duo Multi-Factor Authentication

 

 

 

NOTE: If you have not configured Duo Mobile for Multi-Factor Authentication please complete the steps in the next section.

 

 

 

 

 

 

Congratulations you have now logged into Dynamic Forms. 

 

 

Configuring Duo Mobile:

 

If you have never set up Duo Mobile follow the steps bellow to configure Multi-Factor Authentication. 

 

 

 

 

 

 

1.       Download the Duo Mobile app on your cell phone from the App Store (iOS) or Play Store (Android). 

 

 

 

 

 

 

2.       Open Duo Mobile on your phone and press the ‘Continue’ button. 

 

 

 

 

 

 

 

 

 

3.       Press the ‘Use a QR code’ button. 

NOTE: You may need to allow the app access your camera in your permission settings. 

 

 

 

 

 

 

 

 

 

 

 

4.       On the Dynamic forms login screen click the ‘Duo Mobile’ log in option.

Enter your Phone Number and click the ‘Continue’ button

 

 

 

 

 

 

 

 

7.       Scan the QR Code from the Duo Mobile app on your mobile device.  

 

 

 

 

 

 

 

 

 

 

8.       Designate if you are the only  user on this device.

 

 

 

9.       Now upon logging into Dynamic Forms, you will be able to Multi-Factor Authenticate by pressing the green ‘Approve’ icon in the Duo Mobile App. 

Congratulations! You have now set up Duo Mobile for Multi-Factor Authentication.
