This Knowledge Base article will show you how to fill out a Property Loan Agreement. 

The Employee Equipment Checkout Form Location Knowledge Base article can help you get to the form. 

1) Once you have accessed the Property Loan Agreement form, select your campus from the drop down menu.

2) Input your personal information in all of the required text fields.

3) Type in the reason as to why the item/equipment is being checked out to you in the "List Requested equipment and explain business purpose(s)" field. 

4) When Filling out the dates make sure that the TO (Checkout end of date) is 6/30/20XX, for the end of the Current Fiscal Year. 

5) Ensure that the checkbox for the "Check here if this is a renewal:" is NOT clicked. If this is a renewal please reference the knowledge base article for Employee Laptop Renewal Procedure

6) Read the Terms and Conditions, before signing the Property Loan Agreement.​​​​

7) Click the (Click to sign) field. 

8) Type your first and last name in the appropriate fields and click Sign Electronically

9) Submit the renewal Property Loan Agreement by selecting the Submit Form  button. 

 

 

Your form is going to need to go through a process to acquire signatures to be fully approved. Your supervisor will be one of the people who needs to sign the form. Once it has been approved and the laptop has been prepared you will receive an email letting you know it is ready for pickup.
